Establish a “transitional care” team that is responsible for triaging all new short-term rehab admissions. The goal is to identify those individuals who are not likely to rehab well enough to go home after their Medicare eligibility runs out. Then have social services begin working with the patient and the family members to establish a relationship and build trust with both the patient and the family.
